An opportunity has arisen for an experienced Practice Nurse to join our excellent clinical team at Lawn Medical Centre in Swindon. We are looking for a registered nurse with who can take on the practice’s Diabetic work from our retiring experienced Diabetes nurse. Experience of manipulating medication and sub-optimal control of patients would be desirable.

Key responsibilities would include:

  • Providing a high standard of clinical care
  • Particular management of Diabetes (additional chronic disease management also welcome)
  • Dressings/wound care
  • Childhood Imms
  • Cervical Screening
  • Delivery of holistic patient centred care
  • Lead, develop and implement practice policies

Skills & Experience

  • Proven post registration experience in the following areas:
  • Working autonomously with some leadership responsibility
  • Minor Illness/Triage
  • Chronic Disease Management ideally with an interest in Diabetes
  • Wound care
  • Vaccinations & Immunisations

Qualifications

  • Registered Nurse with current NMC registration
  • Evidence of Continuing Professional Development
  • Prescribing qualification (desirable)
  • Chronic Disease qualification in Diabetes

If you commence employment after the 1st June 2020 you will be employed by the new organisation. If you are employed before the 1st June 2020 you will automatically transfer across to Wyvern Health Partnership on the 1st June 2020.
